# Do I have to use the Assessed Person’s name on the form?

If you wish NOT to use the person's name in the PERSONAL INFORMATION section, leaving it blank is an option. The system will use the ID number in place of the client's name. Other choices are to put in initials, numbers or other organizational codes for the assessed person.
