# How do I print a paper behavioral form in Aseba-Web?

Please click on  the **SELF-PRINTED FORM** tab at the top of the page. Select **SELF-PRINTED FORM **again.

Select the **Culture.**Click **Next**.

Only the forms available in that language will be shown. Select the form and click**Next.**

Fill in the required information on the person to be assessed (initials or numbers can be used to avoid PHI showing).  Evaluation ID is used by hospitals for electronic medical records, this can be skipped.  Click**Next**.

Review the charge for printing a blank form (1 e-unit. Click on **Create Self-Printed Form**.

The form will be created in Adobe pdf format and will downloaded to the Downloads folder. Please open the form and press the print icon. The example below is a French (Canada) CBCL 6-18 behavioral form.
