How do I add a form? (PC and Web)

**Please note, this will be the same for ASEBA-PC and ASEBA-Web.

To add a form, you must first add the assessed person. Please see our How to Add an Assessed person video here:

Once you have added the assessed person and clicked on their name, you will be able to add a form for them. To add the form, simply click the "forms" dropdown at the top of the page, or click the "add form" button located in the center of the page. Please also see the following video:

*Please note: you must click the assessed person's name on the left hand side, then you will see the option for "add form". If you do not see the "add form" button, then no assessed person has been selected.
